In-Home Rehabilitative Services
In-home rehabilitative services are often ordered by the primary care physician following an inpatient stay as a continuation of your recovery and to provide a smooth transition to home and community. Grane Home Health PA offers in-home rehabilitative services of in home physical therapy, in-home occupational therapy, and in-home speech therapy in addition to the RN case manager.
Rehabilitation follows most joint replacement surgeries and strokes and is commonly prescribed for patients with arthritis, neuromuscular diseases, frequent falls, and general debilitation. If you are returning to your own home or an assisted living setting, you may be eligible to continue the in home rehabilitation services offered by Grane Home Health Care. Your RN case manager will assess your medical status, social situation, and functional abilities in your home or the community living setting. Following the assessment, your RN case manager may recommend in-home physical therapy, in-home occupational therapy, and in-home speech therapy, or any combination of the three. A more specific and detailed assessment will then be completed by the therapist, focusing on functions such as walking, bathing, dressing, or communicating. Therapists, after consultation with the primary care physician or specialists, may recommend specific equipment to assist you in carrying out activities of daily living.
